Boris Johnson pointed out that Sue Gray had welcomed changes which have been made to the leadership structure in No 10 in recent months.

The premier said “the entire senior management has changed, there is a new chief of staff” and there is also a new director of communications and a new principal private secretary.

A Labour MP could be heard shouting: “Everybody else’s fault”.
