Some crew members walked off the set of Alec Baldwin’s movie Rust over safety concerns hours before a crew member was shot dead.

Cinematographer Halyna Hutchins was killed and director Joel Souza injured in the incident.

Court records show an assistant director unwittingly handed Baldwin a loaded weapon and told him it was safe to use.
